Table of Contents

First understand PTSD and standard care

PTSD can follow a traumatic event such as violence, disaster, an accident, or combat. Symptoms often include intrusive memories, nightmares, avoidance, feeling on guard, and negative mood or thinking. Not everyone who faces trauma will develop PTSD. Many people get better with time and support. Those who keep having symptoms may need treatment. (National Institute of Mental Health)

The first line treatments for PTSD are trauma focused psychotherapies. Examples include prolonged exposure, cognitive processing therapy, and eye movement desensitization and reprocessing. Large federal guidelines recommend these therapies before medication because they give the strongest and most lasting relief for many people. (U.S. Department of Veterans Affairs PTSD)

Medicines can help too. Some antidepressants can reduce PTSD symptoms, and they may be used when talk therapy is not enough or not available. Your care plan should be personal and based on your goals, your history, and safety. (AAFP)

What is TMS

Transcranial magnetic stimulation uses a magnetic coil placed on the scalp to send brief pulses that activate brain circuits involved in mood and thinking. It is noninvasive and does not require anesthesia. Most people sit in a chair during a brief session and can return to normal activities after the visit. (National Institute of Mental Health)

In the United States TMS is cleared by the Food and Drug Administration for depression and for obsessive compulsive disorder. It is not cleared for PTSD at this time. Researchers are studying how TMS may help PTSD on its own or together with trauma therapy. Some early studies in veterans combine TMS with written exposure therapy and aim to improve outcomes for difficult cases. (National Institute of Mental Health)

Common side effects include mild scalp discomfort and headache during or after a session. The overall safety profile is strong when trained teams follow expert guidelines. (National Institute of Mental Health)

What is Spravato

Spravato is the brand name for esketamine nasal spray. It is a fast acting treatment for adults with treatment resistant depression and for adults with major depression and acute suicidal thoughts or behavior. It must be given only in a certified clinic with at least two hours of monitoring after each session. This safety program is called a Risk Evaluation and Mitigation Strategy or REMS. (FDA Access Data)

Spravato is not approved by the Food and Drug Administration for PTSD. It may still be considered when major depression is part of the picture and you meet the approval criteria. Your clinician will review benefits and risks with you and will check for medical conditions or medicines that may not be safe with Spravato. (FDA Access Data)

When to consider TMS or Spravato for PTSD

You may consider TMS or Spravato for PTSD when one or more of the points below fit your story. Always decide with a licensed clinician who knows your health history.

  1. You tried trauma focused therapy and still have strong symptoms or cannot complete therapy despite support. (U.S. Department of Veterans Affairs PTSD)

  2. You have PTSD with depression that does not respond to standard medicines. In this case TMS or Spravato may target the depression part and help you engage in therapy. (National Institute of Mental Health)

  3. You cannot take more medications because of side effects or medical reasons. TMS is non drug and may be a fit. (National Institute of Mental Health)

  4. You need a structured clinic setting with close monitoring because of safety needs during acute depressive episodes. Spravato provides this setting by design. (FDA Access Data)

  5. You want a care plan that pairs brain based treatment with continued therapy for trauma. Research is testing TMS together with written exposure therapy for better outcomes. (mirecc.va.gov)

What to expect from a TMS plan

  1. Evaluation and mapping
We review your history and past treatments and confirm goals. If TMS is a fit we plan coil placement over the targeted brain region and set an initial dose based on your motor threshold. Sessions usually occur five days a week over several weeks. (National Institute of Mental Health)

  2. Sessions
A trained technician places the coil on your scalp. You feel tapping on the head while the machine runs. Sessions are often brief. Most people return to work or school the same day. Mild scalp discomfort or headache may occur and often fades. (National Institute of Mental Health)

  3. Follow up and sustain
Your clinician tracks your symptoms and adjusts settings if needed. Some people add maintenance sessions later. We coordinate with your therapist so trauma treatment continues.

What to expect from a Spravato plan

  1. Evaluation
We confirm that you meet approval criteria. We check your medicines, blood pressure, and health conditions. We also explain what to expect during your visit. (FDA Access Data)

  2. In clinic session
You self administer the nasal spray under supervision. Then you rest in a calm space. A clinician checks your blood pressure, breathing, and how you feel for at least two hours. Before you go home we confirm you feel stable and you have a safe ride. Driving is not allowed the rest of the day. (FDA Access Data)

  3. Ongoing care
Spravato is given on a schedule that starts with twice weekly visits and then tapers. We continue your therapy and primary care links. If your PTSD includes strong depression, the goal is to reduce that burden so you can engage in trauma focused care. (FDA Access Data)

Safety notes you should know

TMS side effects are often mild. They include scalp discomfort and headache. The risk of seizure is very low when screening and protocols are followed. Tell your team about any seizure history or metal in or near the head. (National Institute of Mental Health)

Spravato can cause sedation, a feeling of disconnection, and a rise in blood pressure during the visit. You will be monitored on site with trained staff and medical equipment. Spravato is only available through certified clinics in the REMS program to keep care safe. (FDA Access Data)

If you are in crisis now or worry about safety, call or text 988 in the United States or use chat on the 988 Lifeline site. Support is free and available any time. If you are outside the United States, contact local emergency services. (SAMHSA)

How to choose between TMS or Spravato for PTSD

Use this simple path

  1. Start with therapy if you have not yet tried a trauma focused approach. If that is not enough, consider brain based care with your clinician. (U.S. Department of Veterans Affairs PTSD)

  2. If your main need is a non drug option or you cannot tolerate more medicines, consider TMS. (National Institute of Mental Health)

  3. If major depression remains severe and you meet approval criteria for Spravato, consider this monitored in clinic option. (FDA Access Data)

  4. Keep therapy going during TMS or Spravato. Many people do best when talk therapy and brain based care work together.

Bottom line for people in Maryland

TMS or Spravato for PTSD can be part of care when standard therapies are not enough. TMS is not cleared by the Food and Drug Administration for PTSD and is used off label after careful review. Spravato is approved for depression, not PTSD, and must be given in a certified clinic with on site monitoring. Our team will help you weigh options with clear education and a plan that centers your goals.
